THE FAMILY of a young boy who died from head injuries has received an apology because a serious case review report was not discussed with them before it was published.

Oxfordshire Safeguarding Children Board (OSCB) released an updated report on Thursday on the investigation into the death of the toddler, known as ‘Child Y’, in 2010.

Tayler Mason, who is believed to be the ‘Child Y’ named in the report, died at the John Radcliffe Hospital in November 2010, aged 22 months.

His father Craig Mason, then 33, of Middleton Cheney, was jailed for 15 months in May last year for neglect. This was later reduced to 10 months on appeal.

An inquest in March heard the toddler had died after falling off a kitchen worktop.

A report of the serious case review in December last year highlighted poor information sharing between police, doctors and social workers and said that the mother’s history of self-harm and abuse from a former partner was not fully considered.

The updated report states that details of the mother’s past should not have been included and apologises for not discussing the review findings before publication.
